Achilles tendinopathy
Achilles tendinopathy is pain and stiffness in the Achilles tendon, usually from overload, and it is common in runners and active people.
It responds well to a progressive loading programme, but getting the load right matters, which is why guided rehab beats guesswork. Complete rest is usually not the answer.
Common symptoms
- Pain and stiffness at the back of the heel or lower calf, often worse in the morning
- Pain that warms up with activity then returns afterwards
- Tenderness or thickening along the tendon
Common causes
- A spike in running volume, speed or hills
- Reduced calf strength or ankle range
- Footwear or surface changes
How it's treated
Treatment starts by identifying which part of the tendon is involved, since that changes the plan, then building a progressive loading programme matched to your stage and your sport. Footwear, temporary heel support and load management round out the plan. Orthotics play a smaller role here and are used selectively.

Should I rest completely?
Usually not. Achilles tendons generally respond better to the right load than to full rest. You'll get guidance on the right level.
How long does it take?
Tendon rehab often takes a few months of consistent loading. Sticking with the programme is the key.
Is it the same as Achilles tendinitis?
The current term is tendinopathy, which better describes what is happening in the tendon. Treatment focuses on progressive loading either way.
